I'm sure this was quite shocking in 1980. When PVC reissued the album in 1987, they decided to play it safe with this:
The original artwork was restored when Triple X reissued the album again in the 1990s.

The old gorehound in me back then would've raised hell about it.
I remember some story about them making their first tour to the east coast. They were somewhere and some teenage girls made them a cake with the original artwork done in icing on it.
